Weekend Referral/Intake Specialist

This is a remote position. Ideally the candidate resides in the Tucson area but not required.

Applicant MUST have home health intake experience!

Are you a self-starter with a passion for patient care and strong attention to detail? We're looking for a Weekend Referral/Intake Specialist to join our dynamic and growing home health agency. What You'll Do

1. Manage weekend referrals from start to finish - review, scrub, verify, and coordinate
2. Ensure compliance with PDGM guidelines, insurance requirements, and intake policies
3. Obtain initial service authorizations and validate coverage
4. Communicate with referral sources and clinicians to set patients up for seamless weekend admissions
5. Support patients as they transition into home care

Schedule

8-10 hours each weekend (flexible based on referral flow)

Key Responsibilities

1. Process and manage weekend referrals in accordance with standards
2. Ensure accurate and compliant documentation and coding
3. Collaborate with referral partners and field staff to meet patient needs
4. Maximize reimbursement through insurance verification and prior authorization
5. Coordinate with providers to initiate care plans
6. Be a knowledgeable point of contact for resources within and outside our agency

What We're Looking For

1. Minimum 6 months experience in a home health intake role
2. Strong communication and coordination skills
3. Ability to work independently and prioritize tasks
4. Bilingual a plus
