Job summary

EverTrue is seeking an Intake LPN for Home Health to process referrals, determine eligibility, and initiate admission processes. You will communicate with patients and referral sources, review clinical documentation, and coordinate with clinicians to ensure timely start of care.

The role requires current LPN licensure in MO or IL, CPR/BLS certification, and prior Home Health or Hospice nursing experience, with reliable transportation and solid computer skills. Part-time, weekends.

Qualifications

• Current Missouri or Illinois State LPN License as appropriate.
• CPR BLS (basic life support) certification.
• Minimum of one year of Home Health or Hospice experience.
• Minimum of 2 years nursing experience.
• Ability to provide own transportation.
• Proficient in computer skills.

Responsibilities

• Answers telephone referrals promptly and with clear communication.
• Elicits accurate information from referral sources.
• Determine valid Face to Face encounter for referrals.
• Enter initial information to begin admission process.
• Notify clinicians for timely start of care or resumption of care.
• Enter physician orders for initial visits.
• Contact patients to inform start of care date.
• Pre-screen patients for Home Health eligibility.
• Document information on unopened referrals.
• Enter insurance authorizations as needed.
• Track follow-up on hospitalized patients.
• Report weekly census to Marketing Office.

Summary:

Responsible for processing new referrals; determining appropriate coverage; communicating with patients and referral sources; and reviewing clinical documentation at Start of Care.

Essential Duties and Responsibilities:

Answers incoming referral calls in a timely manner and with positive communication skills

Elicits accurate, thorough information from referral sources

Responsible for determining valid Face to Face encounter on all referrals

Inputs initial information into computer to begin the admission process

Notifies appropriate clinicians for timely start of care/resumption of care

Enters physician orders for initial visits

Contacts patients by phone to inform them of start of care date

Pre-screens patients over phone for eligibility for Home Health services

Documents information on referrals not opened

Obtains and enters insurance authorizations as necessary

Tracks and provides follow up on hospitalized patients

Reports weekly census to Marketing Office
