Job summary

Lutheran Senior Services is seeking a part-time Home Health Referral LPN to manage weekend referrals and coordinate Starts of Care. The ideal candidate will have an active LPN license in Missouri or Illinois, CPR/BLS certification, and nursing experience in home health or hospice.

Responsibilities include processing referrals, validating eligibility, communicating with patients and referral sources, and entering data to start care promptly. Flexible weekend hours offered.

Qualifications

• Current Missouri or Illinois LPN license as appropriate.
• CPR/BLS certification required.
• Minimum of one year Home Health or Hospice experience.
• Minimum of 2 years nursing experience.
• Ability to provide own transportation.
• Proficient in computer skills.

Responsibilities

• Process new referrals and determine coverage and eligibility.
• Communicate with patients and referral sources regarding Starts of Care.
• Review clinical documentation to support timely admissions.
• Enter referral information into the system to begin admission.

We can work with you to determine a schedule that fits the hours you are able to work each weekend.SummaryThe Home Health Referral LPN is responsible for processing new referrals, determining appropriate coverage and eligibility, communicating with patients and referral sources, and reviewing clinical documentation to support timely Starts of Care.Essential Duties and Responsibilities* Answer incoming referral calls in a timely and professional manner.* Gather accurate and thorough information from referral sources.* Review referrals and determine whether a valid Face-to-Face encounter is present.* Enter referral information into the computer system to begin the admission process.* Notify the appropriate clinicians to coordinate timely Starts of Care and Resumptions of Care.* Enter physician orders for initial visits.* Contact patients to communicate their anticipated Start of Care date.* Pre-screen patients by phone to determine eligibility for Home Health services.* Document information for referrals that are not opened.* Obtain and enter insurance authorizations as needed.* Track hospitalized patients and provide appropriate follow-up.* Report weekly census information to the Marketing Office.Qualifications, Knowledge, Skills and AbilitiesTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily_*Qualifications, Knowledge, Skills and Abilities:*_To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
