Job summary

Hospice of the Piedmont, Inc. is seeking an RN Hospital Liaison to assess eligibility, coordinate hospital discharge planning, and facilitate referrals and admissions to hospice services. This role supports business development and maintains a strong hospital presence to increase referral volume.

The ideal candidate has an RN license in NC, at least two years of clinical nursing, and a willingness to work a flexible schedule including weekend coverage. CHPN is preferred and driving is required.

Qualifications

• Graduate of accredited school of nursing – Associate Degree; BSN preferred.
• Two years clinical nursing experience.
• Current North Carolina Registered Nurse license.
• Valid NC driver’s license, reliable transportation and insurance.
• CHPN (preferred).

Job description

Seeking candidates available to work every Saturday and Sunday. Flexible scheduling options available, ranging from part‑time weekend coverage to full‑time hours with additional weekday shifts.

The Hospital Liaison (RN) is primarily responsible for assessing patient eligibility, coordinating hospital discharge planning, and facilitating timely referrals and admissions to hospice services. This position plays a key part in business development and relationship management and will maintain a strong presence within the hospital and other referral sources to foster connections and increase referral volume.

• Assists with referral intake process by providing program information to prospective patients, families, and referral sources. Provides information, collects information, and develops the referral according to agency procedure. May be involved in the election statement signing.
• Develops and maintains a hospice presence in hospitals, long‑term care facilities, and other targeted referral sources by coordinating with providers, case managers, and other staff that have direct involvement in the referrals and admissions process.
• Evaluates patient’s appropriateness and eligibility for program services. Communicates with primary physician regarding assessment findings and initiation of treatment plan. Nurse will communicate with other members of the interdisciplinary team in order to introduce the patient.

Job Requirements

• Graduate of accredited school of nursing – Associates Degree; BSN preferred

• Valid NC driver’s license, reliable independent transportation, and current automobile liability insurance at limits required by the agency
• CHPN (preferred)
