Job summary

Qualifications

• Graduation from an accredited school of nursing with current compact RN license.
• 1–2 years of varied nursing experience; hospice experience preferred.
• Ability to use technology (EMR, telecommunication tools) in clinical practice.

Responsibilities

• Document calls and follow up if a visit is not scheduled.
• Assist with preliminary assessment of referral information for ordering equipment or meds.
• Communicate with interdisciplinary team promptly and effectively.
• Complete RN Referral Template data collection using required forms.
• Provide telephonic nursing interventions to expedite care and triage as needed.
• Coordinate ordering of durable medical equipment and supplies.
• Maintain licensure and continuing education required for each license.
• Work weekend shifts (Saturday and Sunday) and adapt to scheduling changes.
