Job summary

Liberty Hospice is seeking a dedicated Registered Nurse for weekend per diem coverage to provide high-quality hospice care to patients and support to families in our community.

The role requires current RN licensure and experience in clinical nursing, with hospice knowledge valued. You will collaborate with a multi-disciplinary team, ensure reimbursement-compliant documentation, and participate in routine quality reviews while traveling as needed to patient sites.

Qualifications

• Current RN licensure in state of practice.
• Two years of clinical nursing experience.
• At least one year of hospice nursing experience preferred.
• Knowledge of home health and hospice care framework.
• CPR certification.
• Ability to travel as needed; weekend/evening/holiday call.

Responsibilities

• Apply a working knowledge of nursing theories and concepts.
• Responsible for care and documentation of care which meets reimbursement guidelines.
• Coordinate physical care of the patient with family, patient, physician and Hospice team.
• Communicate with patients/families at all levels.
• Communicate with all agency personnel.
• Attend and participate in IDT meetings and quality review as scheduled by agency.
