JourneyCare Hospice is seeking a Registered Nurse on call for weekends to support patients and families across Deerfield, Northbrook, Des Plaines, and Glenview, IL. You will collaborate with the interdisciplinary team to coordinate care and ensure smooth episode transitions.

The role emphasizes clinical judgment, compassionate care, and rigorous record-keeping to maintain excellent patient outcomes and family support in a hospice setting.

Qualifications

• Graduate from an accredited registered nursing program.
• Current license to practice in the state as a Registered Nurse.
• Minimum of two years of varied work experience as a professional nurse.
• Must possess current CPR certification.
• Valid driver's license and proof of insurance required.

Responsibilities

• Obtain data on patient and family needs and coordinate with the interdisciplinary team.
• Initiate communication with attending physician and other staff for optimal care.
• Perform clinical pronouncement of death and support to family.
• Maintain accurate clinical records and care plans.
