Qualifications

• Must hold an active Colorado RN license.
• Knowledge of pain and symptom management required.
• Strong physical assessment skills and bedside care abilities.
• Proficiency in reading, writing, speaking English.
• Experience with hospice care is preferred.

Responsibilities

• Perform in-person patient visits and triage requests.
• Assess patients using vitals,History and physical findings; report changes.
• Administer medications, provide wound care, manage symptoms.
• Educate patients and families about disease trajectory and expectations.
• Complete charting for Medicare/State agencies within 24 hours.
• Coordinate with care team and other caregivers to arrange services.
• Attend monthly trainings and required in-services.
