Job summary

Suncrest Hospice is looking for a Weekend Certified Nursing Assistant/Home Health Aide (CNA/HHA) to provide compassionate care to residents under the supervision of the Director of CNA Services. The role involves visiting end-of-life patients on weekends, delivering supplies, and triaging as needed.

The position requires CNA and HHA certifications, reliable transportation, and the ability to work well with a multidisciplinary team in a supportive hospice environment.

Qualifications

• Certified Nursing Assistant (CNA) certification required.
• Home Health Aide (HHA) certification required.
• Able to work weekends and drive to patient visits.
• Strong teamwork with nurses, social workers and chaplains.

Responsibilities

• Provide resident care to assigned residents per assessments and care plans.
• Coordinate with nursing staff to deliver appropriate care.
• Follow company nursing policies, safety guidelines and procedures.
• Make weekend visits to end-of-life patients (Saturdays and Sundays) and perform supply delivery and triage visits when needed.
