Qualifications

• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS with hands-on CPR certification.
• 1 year of home health experience preferred.

Responsibilities

• Conduct thorough assessments of patients and their families to determine home care needs.
• Provide comprehensive physical assessments and document the history of illnesses.
• Develop and update individualized care plans with patient involvement.
• Identify discharge planning needs and ensure they are addressed.
• Offer counseling to address nursing and related health needs.

Skills

Current and valid state RN license

Experience in acute care

Basic Life Support (BLS)

Driver's license and auto insurance

Education
