Job summary

Visiting Nurse Association of Hanover & Spring Grove is seeking a Weekend Registered Nurse (RN) to join our Home Health team. The role requires working 12-hour shifts on Saturdays and Sundays, with weekday flexibility offered for personal commitments.

You will assess patients in their homes, develop plans of care, provide skilled nursing services, and collaborate with a multidisciplinary team to ensure high-quality, patient-centered care. Experience in home health and hospice is a plus.

Qualifications

• Current RN license in Pennsylvania (PA).
• Strong clinical assessment and critical thinking skills required.
• Excellent communication and interpersonal abilities.
• Ability to work independently while collaborating with a team.
• Familiarity with EMR systems and home health regulations preferred.

Responsibilities

• Perform initial and ongoing assessments of patients in their homes.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing services including wound care, IV therapy, and medications.
• Coordinate with the care team and refer to other services as needed.
• Document accurately and comply with Medicare/agency policies.
