BAYADA Home Health Care, Inc. in Ohio is seeking a Registered Nurse (RN) to join our home health visit team on a full-time weekend admissions schedule.

You will provide skilled nursing and rehabilitative care to adult and geriatric clients in their homes, enabling them to remain at home with compassionate, high-quality care. The role emphasizes independent decision-making, assessing patient conditions, and coordinating care with a multi-disciplinary team in a Northeast Ohio service area.

Qualifications

• A current nursing license in good standing in the state of Ohio.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).

Responsibilities

• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Provide skilled nursing and rehabilitative care in the home for adult and geriatric clients.
• Triage and manage urgent clinical situations independently in the home setting.
• Conduct medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.
