Humana Inc. is seeking a CenterWell Home Health Weekend Baylor Registered Nurse to deliver one-on-one care in patients' homes, collaborating with physicians and clinicians to support recovery and independence.

You will administer medications, perform wound care, educate families, supervise Home Health Aides, and document care; a valid RN license and CPR are required. The role includes a 40-hour work week with a pay range of $77,200 to $106,200 per year.

Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including meds and wound care.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records of patient care and coordinate with clinicians.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
• Apply your skills to make a positive impact.
