Humana Inc's CenterWell Home Health seeks a dedicated Home Health Registered Nurse to provide one-on-one, in-home care, manage care plans, and help patients regain independence in the comfort of their own homes. You will collaborate with physicians and a team of clinicians to tailor treatment and monitor progress.

This full-time role, 40 hours weekly, requires an active RN license, CPR certification, and a valid driver's license. Competitive benefits are offered by Humana.

Qualifications

• Current and unrestricted Registered Nurse licensure.
• Med-surg, ICU, ER, or acute care experience.
• Current CPR certification.
• Experience collaborating with healthcare professionals.
• Valid driver's license with reliable transportation.

Responsabilités

• Provide skilled nursing care in patients' homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records and coordinate with other healthcare professionals.
• Report progress to the physician and Clinical Manager.
• Oversee Home Health Aides and verify documentation.
