Job summary

Elara Caring is seeking a Registered Nurse for weekend home health coverage. The role emphasizes admissions responsibilities with flexible scheduling (8-, 10-, or 12-hour shifts) and a Friday–Monday focus.

Required: active RN license, 1+ year home health experience, and ability to travel up to 50% with a valid driver’s license. HomeCare HomeBase experience and OASIS knowledge are a plus; base salary determined by factors and a $7,500 sign-on bonus is offered.

Qualifications

• Current, unrestricted RN license in the state.
• 1+ year of clinical nursing experience.
• Willingness to travel up to 50% of the time.
• Experience with Homecare HomeBase (HCHB) and OASIS preferred.

Responsibilities

• Assess caseload of home health patients and plan care.
• Implement, monitor and evaluate the plan of treatment.
• Communicate findings to the supervisor and care team.
• Reassess patients during visits and adjust care plans as needed.
