Job summary

VitalCaring Group is seeking a PRN Weekend Home Health Registered Nurse to lead care in patients' homes. You’ll perform Starts of Care, coordinate with physicians, and guide discharge planning on weekends, delivering high-quality nursing with autonomy.

You will assess needs, adjust plans, and educate families while building strong patient relationships in a supportive, team-oriented environment. Driving to patient homes is required.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Graduate of an accredited nursing program.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
