Graham Healthcare Group is seeking a Weekend Registered Nurse (RN) for Home Health in West Elizabeth, PA. The role delivers skilled nursing care to patients in their homes, coordinating with an interdisciplinary team and traveling within the assigned territory.

This position offers a $10,000 first-year success bonus and a comprehensive benefits package including medical, dental, vision, PTO, 401K, and more. A PA RN license and weekend availability are required.

Qualifications

• Active Registered Nurse license in Pennsylvania.
• Minimum 1 year of RN experience.
• Valid driver's license required for travel.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, document thoroughly in EMR, and ensure compliance.
• Work independently, with travel to patient homes within territory.
