Responsibilities

Deliver skilled, patient-centered care in home health in collaboration with an interdisciplinary team
Perform patient assessments, medication management, and care planning
Educate patients and families on disease management and treatment plans
Ensure accurate, thorough EMR documentation
Travel to patient homes within assigned territory
