Qualifications

• Active Florida RN license is required.
• Driver’s license with reliable transportation is required.
• Home health experience preferred but not required.

Responsibilities

• Conduct comprehensive in-home patient assessments and develop individualized care plans.
• Provide skilled nursing care and caregiver education.
• Administer treatments and monitor patient conditions.
• Coordinate care with physicians and interdisciplinary teams.
• Document accurately and timely using streamlined tools.
