Job summary

BAYADA Home Health Care is seeking a Registered Nurse to join our home health visit team delivering skilled nursing and rehabilitative care in Newton, Dedham, Canton, and surrounding towns. This role emphasizes independent clinical decision-making, comprehensive assessments, and high-quality home-based care to help clients remain at home.

The RN will triage urgent situations, perform wound care, medication reconciliation, and documentation, while collaborating with a multidisciplinary team.

Qualifications

Current Massachusetts RN license in good standing.
Graduation from an accredited nursing program.
At least 1 year of adult/geriatric care experience in acute, home health, or rehab settings.
Medicare Home Health experience including OASIS is required.

Responsibilities

Conduct comprehensive head-to-toe assessments and identify condition changes.
Triage and manage urgent home-based clinical situations independently.
Perform wound care, medication reconciliation, and safety monitoring.
Coordinate care with multi-disciplinary team and document findings.
Prioritize visits and manage a dynamic schedule, ensuring timely patient care.
