Job summary

Interim HealthCare - Farmington, CT is seeking a Weekend Home Health Licensed Practical Nurse (LPN) for a 30-hour position in the Greater Hartford Area. This role focuses on one-on-one home care, coordinating with an RN and health team to deliver quality, compassionate care.

You will take vitals, perform assessments, document progress, and educate patients and families on care plans, medications, and home safety. Must be CT-licensed LPN with CPR and knowledge of home health regulations.

Qualifications

• Graduate of an accredited practical nursing program and active LPN license in CT.
• Minimum of 1200 hours of LPN experience, ideally in home healthcare, or completion of office’s preceptor program.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills and compassion.

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN.
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care.
