Qualifications

• Ability to read, write, speak, and understand English & Spanish.
• Experience working with individuals with developmental or physical disabilities preferred.
• Flexibility to accommodate participant schedules, including evenings or weekends as needed.

Responsibilities

• Provide habilitation supports according to each participant’s Community Habilitation Plan.
• Assist participants in identifying personal goals, preferences, strengths, and learning needs.
• Complete accurate and timely session notes and documentation.
