You will provide direct patient care under RN/LPN supervision, assist with activities of daily living, and monitor vital signs while supporting mobility and documentation.

Qualifications

• High school diploma or equivalent.
• Completion of a state-approved CNA training program.
• Active CNA certification in good standing.
• Strong interpersonal and communication skills.
• Physical ability to lift, move and support residents safely.

Responsibilities

• Provide direct patient care under supervision of a Registered Nurse or Licensed Practical Nurse.
• Assist with activities of daily living such as bathing, dressing, grooming and feeding.
• Monitor and record vital signs (temperature, pulse, blood pressure).
• Assist with resident mobility, transfers and ambulation using proper techniques.
• Maintain a clean, safe and supportive environment and provide emotional support to residents and families.
• Assist in documenting care and report changes in condition to nursing staff.
• Support care teams in developing and implementing individualized care plans.
