Job summary

Beth Israel Lahey Health in Massachusetts is seeking a Part-Time Case Manager for Transitional Support Services. The role focuses on assessments, treatment planning, discharge planning and case management within a patient-centered team.

Requirements include at least one year in addiction treatment, an Associates or Bachelor’s in Human Services or related field, and a valid driver's license. Pay ranges from $21.77 to $26.54 per hour with day-one benefits and weekend coverage.

Qualifications

• Associates or Bachelor’s Degree in Human Services or related field preferred; High School Diploma or Equivalent required.

Responsibilities

• Participates in assessments and aids in formulating effective treatment plans and discharge planning; assists in counseling and case management.
• Assesses continuing care needs; arranges aftercare resources and transfers.
• Attends staff meetings and participates in client evaluations, case conferences and group activities.
• Counsels clients individually and/or in family and in group sessions to achieve treatment goals and monitors progress.
• Refers clients to medical evaluation, social services and employment services as needed.
• Prepares and maintains reports and case histories.

Education
