Job summary

Achieva is seeking a Weekend Coordinator to oversee weekend activities and support for individuals with disabilities in community homes. You will work closely with families, staff, and supervisors to ensure health, safety, and fulfillment for each person served.

The role involves dispensing medications, planning weekend outings, managing personal funds, and maintaining clear communication with the team. Requires PA Act 33/34 clearance, a PA driver’s license, and First Aid/CPR within two months

Qualifications

• Strong interpersonal, communication, and organizational skills.
• PA Act 33/34 clearance required.
• Valid PA driver’s license with properly insured vehicle.
• Ability to perform one-person transfers.
• Must obtain First Aid/CPR certification within two months of hire.

Responsibilities

• Oversee weekend activities, supports, and care for individuals with disabilities living in community homes.
• Dispense and document medications in accordance with policies and procedures.
• Plan and attend approved weekend community outings and medical appointments.
• Manage individual funds and assist with banking as directed.
• Communicate concerns, updates, and activities to the Community Homes Supervisor.
• Ensure safety during crisis situations and follow escalation protocols.
• Maintain effective communication with individuals, families, and team members.
• Perform basic home maintenance and cleanliness tasks.
• Transport individuals in a safe and timely manner.
