Job summary

Innovive Health in Chelsea, MA seeks a dedicated Registered Nurse to deliver skilled home health care. You will assess patients, develop individualized care plans, administer medications and treatments, and coordinate with physicians to ensure safe, high-quality outcomes for patients.

This full-time exempt role includes weekend coverage: Saturday and Sunday 7:00 AM–8:00 PM and Monday follow-ups for documentation.

Qualifications

• Active RN license is required.
• Current CPR certification is required.
• Valid driver's license and reliable transportation are required.

Responsibilities

• Perform patient assessments and develop individualized care plans.
• Deliver skilled nursing care, including medication and treatment management.
• Monitor patient progress and adjust care as needed.
• Educate patients and caregivers on health management and self-care.
• Coordinate with physicians and interdisciplinary teams.
• Supervise Home Health Aides and LPNs.
• Complete timely and accurate clinical documentation (including OASIS).

Schedule (Full-Time, Exempt)

Physical Demands & Work Environment

• Exposure to clinical environments and infectious materials
