Job summary

Sylvan Shores Health & Wellness in La Porte, TX is seeking an LVN/LPN or RN (Charge Nurse) for Saturday and Sunday shifts. This full-time role offers Baylor Pay with day and evening options, supporting resident care, medication administration, and supervision of direct care staff in a compassionate, patient-centered environment.

You will maintain nursing standards, perform rounds, document care, and coordinate with physicians.

Qualifications

• Graduate of an accredited school of nursing (Required).
• Unencumbered RN or LVN/LPN license (Required).
• Proof of current CPR certification (Required).
• Minimum 1 year experience as an RN/LVN in a clinical setting (Preferred).
• Excellent interpersonal skills
• Reliable transportation (Required)

Responsibilities

• Follow established nursing standards and community policies.
• Obtain report from the outgoing nurse and relay to the oncoming nurse.
• Conduct resident rounds daily and report issues to supervisor.
• Identify safety hazards and initiate corrective action immediately.
• Provide supervision to direct care staff and attend meetings as assigned.
• Administer and document medications per physicians’ orders.
• Observe infection control procedures and nursing documentation.
