Hospice of the Western Reserve is seeking a Clinical Admissions Nurse to provide admissions and assessments for hospice services. This role ensures effective communication with referral sources and performs nursing care with compassion and excellence. The position requires nursing experience and a valid Ohio nursing license.
This job operates primarily on weekends with a unique schedule of 24 hours paid for 30 hours of work, ensuring full-time benefits and support for a meaningful approach to end-of-life care.

The Clinical Admissions Nurse is responsible for assuring that HWR programs are effectively and responsibly represented to patients, families, and others who have interest in hospice services through assessment, information sharing and/or admissions in a patient’s home, nursing facility, or in a health care facility. This position works closely with the Referral Department and other referral sources to promote the hospice philosophy of care.
