Job summary

A leading home health agency in California is seeking a part-time Registered Nurse Case Manager in Hayward, Alameda County. Part time Registered Nurse Case Manager

Anticipated Territory : Alameda County: Hayward/Fremont*

Schedule: Saturday/Sunday

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

To be a success:

• Current and unrestricted CA RN license
