Job summary

Avina of Fond du Lac is seeking an Activity Assistant for a part-time, every-other-weekend role to support residents with a well-balanced activity program. You will work under the supervision of the Activity Director to organize, direct and execute engaging activities that meet diverse resident needs across age, ability, interests and background.

Responsibilities

• Directly under supervision of the Activity Director to organize and execute a well-balanced activity program meeting residents' diverse needs.
• Daily visitation on a one to one basis for residents on the One to One program.
• Selection of appropriate programs for residents (crafts, table games, service projects, intellectual activities).
• Balance of small, medium and large group activities across floors and central activity room.
• Balance of recreational, intellectual, religious service and community orientated programs.
• Daily group exercise on floors where feasible; individual exercises when group sessions are not possible.
• Leadership of evening and weekend programs at the direction of the Activity Director.
• Daily recording of resident participation and progress charts quarterly or as needed.
• Participation in the formulation of individual goals with other departments and attendance at care conferences.
• Attend in-service classes and workshops; inform the Activity Director of upcoming needs.
• Contact new residents and familiarize them with activity opportunities.
• Assist in forming monthly activity schedules and evaluating programs.
• Prepare materials and supplies for the day’s programs and notify of upcoming needs.
• Coordinate goals, schedules and programs to support other departments.
