Job summary

Perimeter Healthcare seeks an Activities Coordinator to provide services for social, recreational, physical, educational and spiritual needs of patients and residents. The role emphasizes dignity, empowerment and meaningful engagement through varied activities.

Key duties include assessing interests, writing progress notes, planning events, coordinating calendars, and collaborating with other staff to ensure safe, age-appropriate activities while maintaining high standards of care.

Qualifications

• Must be at least twenty-one (21) years of age or older.
• High School diploma or GED; One year experience in a children's services program or residential setting preferred; a minimum of two years of college education with at least 12 semester (18 quarter) hours in the social sciences may be substituted for one year of work experience.
• Valid State Driver's License with appropriate Endorsements.

Responsibilities

• Assess and reassess activity interests, needs and concerns through observation and written information.
• Write patient/resident progress notes accurately, neatly and timely.
• Record patient/resident codings/changes and other information accurately.
• Plan and choose activities with creativity and variety.
• Notify other departments of changes to scheduled events as necessary.
• Plan calendars with age-appropriate activities for groups or one-to-one sessions.
• Assist patients/residents to participate in activities.
• Adapt activities so residents can succeed with minimal help while maintaining functioning.
• Collaborate with activity staff and other disciplines to provide safe, least restrictive environments.

Education
