Job summary

Beaver Dam Health Care Center in Beaver Dam, WI is seeking a CNA to provide direct patient care and support in a compassionate environment. You will help residents maintain dignity and independence, while guiding them with kindness through daily activities and interactions with families.

Qualifications

  • High school diploma or GED required.
  • Experience working with the elderly or individuals with dementia or physical limitations preferred.
  • Strong interpersonal and communication skills.
  • Patience, creativity, and a compassionate demeanor.
  • Ability to work independently and as part of a team.
  • Basic computer skills for documentation and scheduling.
  • Must be able to lift up to 5 lbs and perform physical tasks such as bending, lifting, and standing for long periods.

Responsibilities

  • Assist in planning, organizing, and implementing daily, weekly, and monthly activity programs tailored to meet the interests and needs of residents.
  • Lead or co-lead group and individual activities, including games, crafts, music, exercise, social events, outings, and religious services.
  • Encourage resident participation and help residents engage based on their individual abilities and preferences.
  • Transport or escort residents to and from activities within the facility and occasionally outside the premises.
  • Document resident attendance and participation; report changes in physical or mental status to the appropriate personnel.
  • Assist in decorating the facility for holidays, birthdays, and special events.
  • Communicate effectively with residents, families, and other staff members.
  • Ensure all activities are conducted in a safe and respectful environment.
  • Support resident-centered care and promote dignity and independence.
  • Maintain activity equipment and supplies, ensuring they are clean and in good condition.

Summary
