Job summary

Beaver Dam Health Care Center seeks a dedicated CNA to provide direct patient care and foster a compassionate environment in Beaver Dam, WI. You will support residents by promoting dignity, choice, and independence while guiding them with kindness through challenging times.

As the first point of contact for residents and families, you will encourage communication, coordinate activities, and contribute to a supportive network that enhances well-being and quality of life within the facility.

Qualifications

  • High school diploma or GED required.
  • Experience in a similar role preferred but not required.
  • Experience with the elderly or dementia/physical limitations preferred.
  • Strong interpersonal and communication skills.
  • Patience, creativity, and a compassionate demeanor.
  • Ability to work independently and as part of a team.
  • Basic computer skills for documentation and scheduling.
  • Must be able to lift up to 5 lbs and perform physical tasks.

Responsibilities

  • Assist in planning, organizing, and implementing activity programs for residents.
  • Lead or co-lead group and individual activities (games, crafts, music, exercise, outings).
  • Encourage resident participation based on abilities and preferences.
  • Transport or escort residents to activities within the facility and sometimes offsite.
  • Document attendance and report changes in status to appropriate staff.
  • Assist in decorating the facility for holidays, birthdays, and events.
  • Communicate effectively with residents, families, and staff.
  • Ensure activities are conducted safely and with respect.
  • Support dignity and independence in resident-centered care.
  • Maintain activity equipment and supplies; keep them clean.

Essential Job Duties
  • Assist in planning, organizing, and implementing daily, weekly, and monthly activity programs tailored to meet the interests and needs of residents.
  • Lead or co-lead group and individual activities, including games, crafts, music, exercise, social events, outings, and religious services.
  • Encourage resident participation and help residents engage based on their individual abilities and preferences.
  • Transport or escort residents to and from activities within the facility and occasionally outside the premises.
  • Document resident attendance and participation; report changes in physical or mental status to the appropriate personnel.
  • Assist in decorating the facility for holidays, birthdays, and special events.
  • Communicate effectively with residents, families, and other staff members.
  • Ensure all activities are conducted in a safe and respectful environment.
  • Support resident-centered care and promote dignity and independence.
  • Maintain activity equipment and supplies, ensuring they are clean and in good condition.
Qualifications
  • High school diploma or GED required.
  • Prior experience in a similar role (preferred, but not required).
  • Experience working with the elderly or individuals with dementia or physical limitations (preferred).
  • Strong interpersonal and communication skills.
  • Patience, creativity, and a compassionate demeanor.
  • Ability to work independently and as part of a team.
  • Basic computer skills for documentation and scheduling.
