You will work with a kind, collaborative care team to ensure safety and comfort, complete accurate progress notes, and communicate clearly with families.

Responsibilities

• Provide personal care, including bathing, dressing, and grooming; Hoyer lift experience preferred.
• Assist with daily living activities such as meal preparation, medication management, and mobility support.
• Promote independence and safety for clients.
• Develop relationships with clients, families, and care teams to deliver holistic care.
• Communicate effectively with clients, families, and staff to ensure seamless care transitions.
• Document client progress and maintain accurate records.
