Job summary

Owenton Healthcare and Rehab is seeking a licensed practical nurse (LPN) to join a compassionate long-term care team in Kentucky. You will provide bedside care, monitor vital signs, administer medications, and communicate with RNs and physicians to ensure quality patient care.

This role emphasizes continuity of care, accurate charting, and a supportive work environment with opportunities for professional growth and staff appreciation programs.

Qualifications

• Current LPN license in Kentucky state.
• One year of experience in long-term care preferred.

Responsibilities

• Provide bedside care to patients including personal hygiene.
• Record patients' vital signs (BP, pulse, respiration).
• Relay information between patients, RNs, and physicians.
• Give oral medications and injections to patients.
• Test patient blood sugar levels.
• Assist patients with daily activities and chart care accurately.
• Maintain confidentiality of patient records.

LPN Responsibilities:

• Provide bedside care to patients including personal hygiene maintenance

• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)

• Collect patient’s health information

• Relay information between patients, registered nurses, and physicians

• Give oral medications and injections to patients

• Test a patient’s blood sugar levels

• Urinary catheter insertion and care

• Assist patients with personal hygiene

• Monitor for changes in patients

• Proper charting, documentation, and maintenance of patient’s medical records

• Asks questions when in doubt for clarification by the Registered Nurse.

• Acts as a role model for ancillary personnel.

• Maintains the confidentiality of the patient’s clinical records.

LPN Requirements:

• Current LPN license in State.

• One year of experience in long-term care is preferred.
