Job summary

HealthOp Solutions seeks a full-time Nurse Practitioner or Physician Assistant to join an outpatient family medicine clinic in Cottonwood, AZ. You’ll provide comprehensive primary care with a focus on continuity, preventive care, and patient-centered outcomes.

You’ll enjoy a Monday–Friday schedule with no weekends or after-hours on-call, a collaborative care team with strong admin and clinical support, including a dedicated medical scribe to streamline documentation.

Qualifications

• Active, unrestricted Arizona license as a Nurse Practitioner or Physician Assistant.
• Current DEA registration.
• Eligible for credentialing with Medicare, Medicaid, and commercial insurers.
• Clean professional record with no history of malpractice or disciplinary actions.

Responsibilities

• Perform comprehensive and focused visits including annual physicals and preventive screenings.
• Diagnose and manage chronic illnesses such as diabetes, hypertension, asthma, and COPD.
• Order and interpret diagnostic tests and imaging studies.
• Develop and implement individualized treatment plans.
• Provide education on disease prevention and healthy lifestyle practices.
• Collaborate with physicians, RNs, and administrative staff.
• Document all clinical encounters in the EHR with scribe support.
• Manage a patient panel of approximately 10–15 patients per day to ensure quality outcomes.
