Responsibilities

• Triage patient needs over the phone until an in-person visit can be provided
• Respond to after-hours crisis calls with urgency
• Perform initial nursing evaluation within 48 hours of referral
• Conduct after-hours visits including admissions and recertifications
• Assess and reassess patient/family needs every 15 days
• Document problems, goals, interventions and responses
