Job summary

Valor Healthcare is seeking a dedicated Licensed Practical Nurse to join the Bowling Green Community Based Outpatient Clinic. Expect a competitive salary, excellent benefits, generous time off and a Monday–Friday schedule.

You will support Veterans through prescribed care, collaborate with the PACT team, and provide clinical services in a clinic setting with a patient-centered approach.

Qualifications

• Diploma or certificate of an accredited practical nursing program.
• Minimum one-year LPN/LVN experience in primary or ambulatory care.
• Current license in state of employment and VA credentialing as applicable.
• BLS and ACLS certifications as required by contract.

Responsibilities

• Provide prescribed medical treatment and personal care per physician direction.
• Collaborate with PACT team and expanded care team to develop patient care plans.
• Administer medications and document in the electronic medical record.
• Perform nursing assessments, vitals, and preventive health services as directed.
• Operate diagnostic equipment (EKG, glucometers, spirometry) as needed.
