Concern Housing in New York seeks a Weekday Evening Counselor to assist the Supervisor and Service Coordinator in delivering direct services to clients, teaching skills and providing counseling and daily living support.

Based at our Brooklyn site, the role runs Monday through Friday from 4pm to midnight, with supervision and staff collaboration to maintain safety, records, meals, activities, and transportation as needed.

Qualifications

• Requires a high school diploma or equivalent and at least two years of experience in a similar role.

Responsibilities

• Provide direct care services to clients.
• Document client progress and program activities.
• Coordinate and participate in site activities (meals, chores, recreation, safety checks).
• Support daily living skills, health services, and rehabilitation activities.
• Monitor medications and prepare medication sheets.
• Assist with transportation of clients.
• Perform unit inspections and help residents with upkeep.
• Attend weekly supervision and staff meetings.
• Maintain required agency compliance documents and trainings.

Description

Concern Housing is seeking a WeekdayEvening Counselor to be responsible for assisting the Supervisor and Service Coordinator in the delivery of direct services to the clients, as well assisting in the overall running of the site. The counselor provides a variety of services, ranging from the teaching of skills to delivery of concrete services to providing counseling to the clients.

Responsibilities Include but are not limited to:

• Provides direct care services to clients.
• Responsible for documentation of clients assigned to as well as documentation relating to other clients progress.
• Monitors the site, coordinates and participates in house activities such as preparation of meals, chore activities, recreational activities, safety checks etc.
• Provide rehabilitative support services in daily living skills, health services and others as assigned.
• Monitors medication; does medication counts; prepares medication sheets.
• Assists with transportation of clients.
• Performs unit inspections, assists residents with upkeep and cleanliness of individual units
• Attends weekly supervision and staff meetings.
• Remain current with all Agency required compliance documents and trainings.

Requirements

Requires a minimum of a high school diploma or equivalent along with at least two years of experience in a similar role.

Salary Range: $41,600
