Montefiore Medical Center is seeking a Review Physician for the WeCARE program in the CMO department in Bronx, NY. The role supports the Medical Director to ensure NYC HRA contractual compliance.

Responsibilities include medical review of assessments, performing medical assessments per NYC HRA protocols, and developing Wellness Plans. Requires MD with NY license; board certification; 2 years of clinical practice; experience with SEAMS, Epic, PSYCKES; strong computer skills.

Qualificações

• MD with unrestricted NYS medical license and board certification.
• Experience in Internal Medicine, Family Practice, PMR, or Emergency Medicine.
• Ability to assist and motivate clients to comply with WeCARE processes.
• Experience with NYC HRA workflows and relevant software.

Responsabilidades

• Conduct medical review of Clinical Assessments/Clinical Reassessments.
• Perform Medical Assessments per NYC HRA protocols.
• Complete Wellness Plans dispositions for WeCARE clients.
• Support other staff to fulfill funder and contractual goals.
