General Summary
With support from the WARM Line Program Coordinator, provides telephone support and assistance with resources and referrals for individuals affected by addiction. Works with WellSpan providers and community stakeholders in five counties to contribute to the coordination, management, and expansion of addiction services for the system.
Duties and Responsibilities
Essential Functions:
- Provides telephone support and connects individuals to community services for addiction resources.
- Facilitates referrals and schedules appointments for MAT for individuals with opioid use disorder (OUD).
- Supports universal screening in primary care offices via SBIRT for risk stratification based on DAST scores.
- Provides ASAM level of care assessments to facilitate treatment referrals.
- Reviews and manages the Recovery Support Department work queue.
- Ensures the WARM Line resource directory is current and accurate.
- Follows and updates the WARM Line Standard Work as needed.
- Participates in continuous improvement efforts for the WARM Line.
- Prioritizes safety, referring/reporting individuals to crisis services, 911, and PA Childline as needed.

- Completes documentation for each telephone encounter and maintains monthly service logs.

Qualifications
Minimum Education:
- High School Diploma or GED, preferably with some college social science credits.
Work Experience:
- Less than 1 year in a health-related field, preferably with individuals with substance use disorder.
Knowledge, Skills, and Abilities:
- Effective verbal and listening interpersonal skills.
- Analytical skills for interpreting data, treatment planning, and evaluating program effectiveness.
- Organizational skills for managing clinical program operations and strategic planning.
