Performs general public health social work in accordance with public health standards and agency practices. The social worker's focus will be care coordination and the provision of referral consultation to the MYAT team at Weld County Department of Social Services. Initial contact through appropriate assessment, culminating in referral to community based agencies and public health programs. Bi-lingual in Spanish preferred. This is a 30 hour per week benefited position.

•Provides appropriate assessment, culminating in referral to community based agencies and public health programs.
•Consultation to MYAT for the purpose of connecting public health services to families.
•Attend MYAT case conferences and team meetings to provide input and advice on programs.
•Function as a liaison between families and the medical community.
•Will have knowledge of appropriate resource agencies and programs within the community.
•Assist families with meeting Social Services time mandates to receive required assessments.
•Awareness of cultural issues which may impact family decision making and desire to access and obtain health/mental health related services.
•Understanding of basic child welfare.
•Ability to do family assessment, identifying needs and prioritizing interventions.
•Awareness of the emotional and psychological issues impacting children.
•Knowledge of basic mental health diagnoses and ability to facilitate appropriate mental health referrals.
•Competency in recognizing the signs of child maltreatment, and appropriate skills in addressing those concerns with caregivers.
•Ability to educate families regarding their rights and responsibilities as consumers of health care.
•Provide home visits as needed/requested by the clients and the team.
Education: A Bachelor's degree in Social Work or other Human Behavioral Services Field.
Experience: A minimum of one year working in public health desired.
A valid State of Colorado driver's license and use of personal insured vehicle is required to perform job duties. Auto insurance as required by state law.
