Qualifications

• Master’s degree or higher in social work, psychology, mental health counseling, or marriage and family therapy.
• Rehabilitation counseling (C.R.C. preferred) if applicable.
• Valid NY State driver license.
• Minimum 1 year of experience in a treatment or rehabilitation setting.
• Fingerprinting and criminal record check.
• Approval from NYS Office of Mental Health.

Responsibilities

• Provide tailored care and guidance to participants and families.
• Function as part of a mobile, multi-disciplinary ACT team in the community and office.
• Apply employment, rehabilitation, and skill-building techniques to support clients’ goals.
• Coordinate with community resources to advance clients' work, education, housing, and social functioning.

RiseWell Community Services is looking for a full-time Master Level Counselor – Vocational Specialist to join our Assertive Community Treatment Program in Medford, New York. In this role, you’ll be utilizing your own professional experience to provide tailored care and guidance to participants and families. Our Assertive Community Treatment teams provide a full suite of treatment to individuals dealing with serious mental illness, so they can get the support they truly need. ACT services aim to prevent relapse, teach valuable life skills, provide assistance, and connect individuals with essential community resources so they can move forward and thrive in work, school, home life, and social relationships. As a part of our ACT team, you’ll be working as a collaborate, mobile, multi-disciplinary treatment team, both in office and in the community. In this role, you will act as a member of a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness, who integrates information and expertise on supported employment, job development, and rehabilitation methods that assist the client with developing skills in his or her environments of choice.

Qualifications
