Job summary

You will extract data from medical records, complete registrations, assist mothers, notarize affidavits, and input data into the state registry, with state-specific training provided.

Qualifications

• High School diploma or GED required.
• Healthcare experience preferred but not required.
• Excellent communication and interpersonal skills.

Responsibilities

• Completes state birth registry worksheet by extracting data from the medical record for birth/death registration.
• Assists mother to complete state birth registry worksheet.
• Completes and notarizes paternity affidavits as required by state law.
• Inputs all required information into the electronic birth/death registry.
• Performs other duties as assigned.
