Job summary

Hartford HealthCare at Home is seeking a Primary Care Nurse (RN) to deliver in-home nursing services across Connecticut. The Visiting RN will assess patients, develop personalized care plans, and partner with an interdisciplinary team to promote autonomy and self-advocacy in the home setting.

The ideal candidate holds a CT RN license, has at least one year of RN experience (homecare preferred), and possesses a valid driver’s license with reliable transportation.

Qualifications

• CT RN license required.
• 1 year RN experience; homecare experience preferred.
• Valid drivers license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Perform comprehensive assessments to establish an individualized care plan.
• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.
