Job summary

Hartford HealthCare at Home in Connecticut is seeking a Registered Nurse (RN) with a valid CT RN license to join our Primary Care team. You’ll perform comprehensive assessments, establish individualized care plans, and coordinate with the interdisciplinary team to deliver patient-centered care in the home.

One year of RN experience is required, homecare experience preferred. A valid driver’s license and reliable transportation are essential, and computer proficiency is expected.
