We are highly skilled nurses who identify home safety deficits, nutrition and hydration concerns, potential medication errors, and assess the impact of social determinants of health. The VNA care team collaborates with patients, caregivers, and ambulatory providers to deliver appropriate services to facilitate outcomes. Whether a patient is recovering from a single acute event or living with a new or chronic illness, nurses provide education, safety recommendations, and hands‑on skilled interventions to help reduce future hospitalizations.
