About the Job:
Delivers home health care to client/patients of all ages and families whose needs range from basic to complex and are rapidly changing by utilizing the nursing process. Creates a therapeutic environment while identifying and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, and the provision of care supportive to or restorative of life and well-being. Practices under the direction of the Clinical Manager and supervision of the RN within the departmental policies and procedures.
What You’ll Do:
Who You Are:
Education: Graduate of an accredited school of Licensed Practical Nursing required.
Experience: Minimum of 2 years previous clinicalexperience required, preferably in a home care setting.
Certification/Licensure: LPN NH licensure required. Current BLS certification or must obtain within 90 days of date of hire required.
Schedule: Work schedule may include eight (8) to twelve (12) hours per day with weekend, evening, and night coverage based on VNA needs. Staff are expected to cover for absences by rotating or working extra time. Staff are assigned to the services area of VNA to meet client/patient needs.
Other: Valid Driver’s License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities. Proof of auto mobile insurance required. Motor vehicle background report will be conducted.
