Job summary

Jobot is seeking a Visiting Homecare RN in the New York City metro area. You will conduct home visits, coordinate care with physicians and other health professionals, and develop individualized plans to promote healthy lifestyles and reduce avoidable hospital visits.

Responsibilities include documentation, medication profile updates, and collaboration with PCPs to ensure continuity of care. Medicaid/Medicare guidelines knowledge is required, and a sign-on bonus is offered.

Qualifications

• RN license (active) required.
• Experience in home health or visiting nurse roles preferred.
• Knowledge of Medicaid/Medicare regulations and guidelines.
• Ability to develop individualized care plans and promote healthy lifestyles.

Responsibilities

• Manage all aspects of patient care in the home setting.
• Conduct home visits and care coordination with other disciplines.
• Collaborate with patients, physicians, RNs and health professionals to ensure quality care.
• Develop individualized care plans and promote healthy lifestyles.
• Complete required documentation and update medication profiles.
• Daily review of follow-up items and documentation in EMR.
