Job summary

Jobot is seeking a Homecare CHHA RN to oversee patient recovery in the Brooklyn NY area. The role includes 5–6 patient visits daily, coordinating care among physicians, therapists and home health staff, and documenting in KanTime EMR.

You must hold a NY RN license and have car access. The position offers a $115,000–$120,000 annual salary, a $15,000 sign-on bonus, 4 weeks vacation, 8 holidays, and a comprehensive pension/retirement plan.

Qualifications

• RN with active New York State Nursing license in good standing.
• Homecare / CHHA experience preferred and OASIS experience highly preferred.
• RN must have a car/vehicle to be considered.

Responsibilities

• Oversee the process of a patient’s recovery from illness, injury or surgery.
• Conduct 5–6 patient visits per day (Mon–Fri, 8:30 am–5 pm).
• Coordinate plan of care among physician, physical therapist, social worker and home health aide services.
• Reconcile medications with the physician and educate patients/caregivers on use, doses and side effects.
• Enter patient visit documentation into the KanTime EMR and manage care transitions.
